MAKING AN APPOINTMENT

To make an appointment, please call our office to schedule.  If no one is available to answer your call, please leave a message and we will return your call within 24 business hours. 

VISITING THE OFFICE FOR THE FIRST TIME

It is recommended you print and fill out your Patient Registration Form prior to your first appointment.  Please arrive 15 minutes early if you need to prepare your forms prior to your appointment.

WHAT SHOULD YOU BRING?

If you have Medicare or any other insurance plan, please bring your insurance card and proof of identity such as a driver’s license. It is very helpful if you write down all medications and supplements (vitamins, health products) you are taking. If relevant, please contact your former health care provider, and request your records be forwarded to our office.

PUNCTUALITY

Please arrive on time for your appointment, and at least 15 minutes early if you need to prepare forms prior to your appointment. We strive to see patients in a timely manner. Timely arrivals by our patients are important for our schedule. If a patient’s arrival is delayed and jeopardizes the schedule of the office, we reserve the right to require the patient to reschedule the appointment.

CANCELLING APPOINTMENTS

If you are unable to make an appointment, please let us know as soon as possible, and no later than 24 hours prior to the time of the appointment. Patients who no show to their appointment, or cancel with less than 24 hours notice are subject to a $50 fee.

MEDICARE AND IN NETWORK INSURANCE 

Houston Institute of Dermatology accepts Medicare, and is in network with most major insurances. Please contact your insurance company to determine whether we are in network for your insurance plan. 

Most insurance plans have deductibles, co-pays, and co-insurance provisions. Patients are expected to pay these fees at the time of the visit. If you have a high deductible health plan, and you have not met the yearly deductible for your insurance plan, you will be expected to pay the full fee at the time of your visit. New or established patients who require a referral will be responsible for obtaining their own referral from their primary care doctor.

If we are not “in network” with your insurance, you are welcome to be seen as a patient. Fees are due and payable at the time of your visit. We will issue you an insurance coded, itemized receipt that you may file with your insurer for reimbursement under the terms of your insurance plan.

In determining fees, we use what your insurance has provided us, and the in network fee for your insurance plan. After the insurance plan processes the claim, the fee approved by the plan may be more or less. In this situation, you may receive a supplemental invoice depending upon how the claim is processed by your plan. In other situations, you might receive a refund or credit due to over payment. A credit may be used to offset future fees due, or may be refunded to you via check.

PATIENTS WITHOUT INSURANCE

We welcome individuals who do not have insurance. We will provide you with a fee estimate in advance of the procedure. Deposits and fees are due and payable at the time of your visit.

COSMETIC DERMATOLOGY

Cosmetic Dermatology is not covered by insurance. Patients with new procedures pay a cosmetic consultation fee first. After the consultation, you will be provided with a fee estimate in advance of the cosmetic procedure. The fee will be reduced by the cosmetic consultation fee. Cosmetic fees are due and payable at the time of your visit.

PAYMENT FORMS ACCEPTED

Houston Institute of Dermatology accepts cash and Visa, MasterCard, American Express, and Discover credit cards. Personal checks are not accepted.
